Test and Trace Support Payment Application Form

See making a Test and Trace Support Payment application for details on who should apply for the Test and Trace Support Payment, and what information you will need to provide.

Privacy notice

If you wish to apply for the Test and Trace Support Payment, North Tyneside Council will access your records contained within the NHS Test and Trace data system - known as CTAS - and the Department for Work and Pensions (DWP) Customer Service Information system (CIS) - known as Searchlight - for the purposes of confirming your eligibility for this scheme.

From 8 March 2021, if you make a claim because you need to take time off work to care for a child or young person who has been told to self-isolate by NHS Test and Trace or by their education or childcare setting because they have been identified as a close contact of someone who has tested positive for COVID-19. Please note, in order to validate your claim we will also check with the child or young person’s education or care setting to verify the self-declared information provided is correct, and that the child or young person did not continue to attend the education or care setting when they should have been self-isolating.

You can find out more about CTAS by visiting the Test and Trace Privacy Notice (opens in a new tab).

North Tyneside Council will use the data provided to process your claim for this funding, to report to NHS Test and Trace, and - because these payments are subject to income tax - to report successful claims to HMRC.

If you wish to withdraw your application you may do so by contacting the council (opens in a new tab) at any time. Please be aware that if you do not provide the required data you will not be entitled to the Test and Trace Support Payment.

North Tyneside Council will carry out checks to prevent fraudulent claims. This may mean contacting your employer to confirm you were unable to work from home. There may be other instances where the council will need to investigate your entitlement to a Support Payment.

If your claim is successful and North Tyneside Council becomes aware, either through post-payment verification checks or through other means, that you have not self-isolated, we will refer your case to the police.

Further information on how North Tyneside Council uses your data can be found in our Privacy notice (opens in a new tab).
